Dr Gray practised in Exeter. Professor of General Practice at the University of Exeter since 1986; Director of Postgraduate Medical School (1987–1997). He established the first postgraduate University Department of General Practice at Exeter University. His submission on the history of this department won the Rose Prize in 2013. He was the only GP ever elected Chairman of the Academy of the Medical Royal Colleges of GB and Ireland. He was Patron of the National Association for Patient Participation and Assessor for Queen’s Award for Higher and Further Education, Editor of 'Forty Years On: the story of the first forty years of the RCGP' (1992), and Chairman of Nuffield Trust (2003). Dr Gray is shown seated wearing a dark pin-striped suit, red tie, Presidential gown chain of office.
